✅ Description:
Itraconazole inhibits fungal cytochrome P450–dependent enzymes, thereby blocking ergosterol synthesis, an essential component of fungal cell membranes. This disrupts membrane integrity, resulting in fungal cell death. It is active against a wide spectrum of fungi, including Candida, Aspergillus, Trichophyton, Microsporum, and Histoplasma species, as well as Leishmania (antiprotozoal action).

✔️ Dosage & Administration of Itracare SB 100
Adults (Oral):
- Oropharyngeal candidiasis: 100 mg once daily for 15 days
- Vaginal candidiasis: 200 mg twice daily for 1 day
- Pityriasis versicolor: 200 mg daily for 7 days
- Tinea corporis/cruris: 100 mg daily for 15 days
- Tinea pedis/manuum: 100 mg daily for 30 days or 200 mg twice daily for 7 days
- Onychomycosis: 200 mg daily for 3 months
- Systemic infections: 100–200 mg daily; up to 200 mg twice daily for invasive infections
- Prophylaxis (neutropenic/AIDS patients): 200 mg daily, may increase to twice daily if needed
Children (Oral):
- General fungal infections: 5–10 mg/kg/day, max 200 mg/day
- Chronic mucocutaneous candidiasis: 5 mg/kg/day
- Under 3 years: Safety and efficacy not established
Instructions:
Take immediately after a full meal. Swallow whole without crushing or chewing.

- Known hypersensitivity to itraconazole or excipients
- Severe liver disease
- Co-administration with rifampin (affects the metabolism of itraconazole)

Avoid concomitant use with the following due to serious risks:
- Terfenadine, astemizole, cisapride → Risk of QT prolongation, torsades de pointes
- Oral midazolam, triazolam → Enhanced sedative effects
- Statins (simvastatin, lovastatin, atorvastatin) → Risk of myopathy/rhabdomyolysis
- Ergot derivatives → Risk of ergotism
- Methadone, quinidine, pimozide, halofantrine, etc. → Cardiac risks
Other interactions:
- Rifampin, phenytoin, phenobarbital, carbamazepine → Decrease itraconazole levels
- Calcium channel blockers (e.g. verapamil, diltiazem) → Risk of edema, heart effects
- Digoxin, cilostazol, repaglinide, corticosteroids → Increased plasma levels
- Antacids, PPIs, H2 blockers → Reduce absorption of itraconazole
- HIV protease inhibitors, clarithromycin, erythromycin → Increase itraconazole levels
- Fentanyl → Risk of respiratory depression
- Colchicine (renal/hepatic impairment patients) → Toxicity risk
- Eplerenone → Risk of hypotension and hyperkalemia

- Absorption is reduced in low stomach acidity; avoid antacids within 2 hours of dosing.
- Use caution in patients with liver disease, heart failure, COPD, or renal insufficiency.
- Monitor liver function during extended use (over one month).

- Use effective contraception during treatment.
